Event Description
It is reported that during a surgery, the impactor broke.During the first contact, the plastic part from impactor broke off.
 
Manufacturer Narrative
An event regarding crack/fracture involving a mako impactor was reported.The event was confirmed.Method & results: device evaluation and results: a visual inspection is performed and found that fracture consistent with an overload condition as indicated by hackles observed on fracture surface.Medical records received and evaluation: no medical records or x-rays were made available for evaluation.Device history review: device history review indicated the devices accepted into final stock from the reported lot were free from discrepancies complaint history review: there has been no other event for the lot referenced.Conclusions: the event is confirmed.A visual inspection of returned device is performed and found that fracture consistent with an overload condition as indicated by hackles observed on fracture surface.If the additional information are received, this investigation will be reopened and re-evaluated.
