Currently it is unknown whether or not the device may have caused or contributed to the event.The device has been returned, but not yet evaluated.Further information will follow once the analysis has been completed.No conclusion can be drawn at this time.(b)(4).The insulin pump involved in this event is the 640g insulin infusion pump, which is not marketed in the united states.However, the device is similar to the paradigm real-time insulin infusion pump, which is marketed in the united states.
 
Event Description
It was reported via phone call that the insulin pump had a flashing white and black display with beeping continuously.The customer¿s blood glucose level was 144 mg/dl at the time of the incident.Customer didn't recall any event leading to the issue.The customer was advised to discontinue use of the insulin pump and to revert to the back-up plan.The insulin pump was returned for analysis.
 
Manufacturer Narrative
The insulin pump was received with an unexpected white flashing display followed by an unexpected intermittent beep alarm due to vertical cracked lcd controller.Unable to perform the self test, displacement test, rewind test, prime/seating test, basic occlusion test, force sensor test, occlusion test, sleep current measurement test, and active current measurement or verify missing segments/partial display test due to flashing display anomaly.The insulin pump was received with scratched case and pillowing keypad overlay.
