Patient identifier: (b)(6).A medtronic representative inspected the imaging system on-site.Hardware investigation was completed.This issue was found related to a hardware issue and was documented in a medtronic hardware anomaly tracking database.The door was jammed, damaging the door winch assembly parts, rotor.All damaged parts were replaced and the issue has been resolved.System functionality was confirmed and the system was returned to service.
 
Event Description
A site biomed representative reported while in a spinal fusion procedure, the gantry on the imaging system could not rotate in order to open the door.When the button was pressed, a thumping noise as if the motor was skipping was heard.An attempt was made to rotate the door manually, but the issue did not resolve.The imaging system was on a closed bed so it could not be lifted and shifted off.The system was moved to the base of the bed and the case proceeded.There was no impact to the patient and there was no reported delay to the procedure due to this issue.
