(cement extravasation) neither the device nor applicable imaging films were returned to manufacturer for evaluation therefore we are unable to determine the definitive cause of event.
 
Event Description
Pre-operative diagnosis for this procedure: primary osteoporosis type of fracture: compression fracture it was reported that on (b)(6) 2017, intra-op, cement leaked to the side of the vertebral body.The product came in contact with the patient.No patient complications were reported as a result of this event.
