This alleged failure mode poses a low risk to a patient because the issue was observed when the freedom driver was not supporting a patient.The freedom driver will be returned to syncardia for evaluation.The results of the investigation will be provided in a follow-up mdr.
 
Event Description
The freedom driver was not supporting a patient at the time of the reported event.The customer, a syncardia certified hospital, reported that the patient's backup freedom driver had a damaged driveline strain relief.
 
Manufacturer Narrative
The freedom driver was returned to syncardia for evaluation.The customer-reported issue of a frayed strain relief was confirmed through visual inspection of the driver.The inspection revealed markings on the driveline under the damaged part of the strain relief.It is likely that the strain relief was struck by something, perhaps when setting the driver down, as there were marks on the drivelines on the underside of the driver.The root cause of the customer-reported issue could not be conclusively determined.This issue will be monitored and trended as part of the customer experience process.Syncardia has completed its evaluation of this complaint and is closing this file.(b)(4) follow-up report 1.
