Event Description
The customer reported via phone call that their insulin pump had a motor error alarm and the drive support cap was flush.The customer¿s blood glucose level was unknown at the time of the incident.The customer reported that the customer took the pump into the magnetic resonance imaging room and received a motor error.They heard beeping while she was in the magnetic resonance imaging room and took her out of the magnetic resonance imaging room.The customer reported that the drive support cap was flush.The customer was advised to discontinue use of the insulin pump and to revert to the back-up plan.The insulin pump will be returned for analysis.
 
Manufacturer Narrative
Pump alarmed motor error during rewind due to motor encoder signal out of phase.Unable to perform the self-test, unexpected restart error test, displacement test, rewind, basic occlusion test, occlusion test, prime test and excessive no delivery test due to motor error alarms.Pump received with minor scratched lcd window, cracked belt clip slot, cracked battery tube threads, stained end cap sticker, stained address/serial number label and cracked reservoir tube lip.
