Suspect device was not returned.Process evaluation did not reveal any production error.Review of the treatment plan showed that the implant was planned lingually and centered in the bone.Free handing the surgery to make it more buccal caused the implant to perforate the buccal bone plate.
 
Event Description
Doctor used a surgical guide to place dental implant.After the pilot drill, doctor thought that the trajectory was very lingual in comparison to how he planned the treatment plan.Doctor freehanded the surgery but ended up perforating the buccal plate instead.
