Device codes: (b)(4).Internal file number - (b)(4).The device was returned and investigated. The reported mechanical issue and cable break were confirmed. The reported noise could not be replicated in the testing environment as the event was a symptom of the cable break.A review of the lot history record revealed no manufacturing nonconformities.Additionally, a review of the complaint history identified no other incidents reported from this lot.The mitraclip nt instructions for use warns, all catheter manipulations should be done with care.Do not continue to rotate or manipulate any of the handle controls if significant resistance is noted. The investigation determined the reported steerable guide catheter cable break, resulting in noise and the inability to curve guide appears to be related to user error due to applying extreme curve on the guide.Based on the information reviewed, there is no indication of a product quality issue with respect to design, manufacture or labeling of the device.
