Event Description
This is filed to report a cable break.It was reported that this was a mitraclip procedure to treat mixed mitral regurgitation (mr) with a grade of 4.After placement of the steerable guide catheter (sgc) on the guide wire, prior to entering the anatomy, the decision was made to apply a little bit more negative on the +/- knob which resulted in a loud noise.The knob was moving freely with no tip deflection at all.A cable break was suspected.The sgc was replaced with a new device and the procedure continued on successfully.Post-mr grade was 3.No adverse patient effects or clinically significant delay in the procedure were reported.No additional information was provided.

Device codes: (b)(4).Internal file number - (b)(4).The device was returned and investigated.The reported mechanical issue and cable break were confirmed.The reported noise could not be replicated in the testing environment as the event was a symptom of the cable break.A review of the lot history record revealed no manufacturing nonconformities.Additionally, a review of the complaint history identified no other incidents reported from this lot.The mitraclip nt instructions for use warns, all catheter manipulations should be done with care.Do not continue to rotate or manipulate any of the handle controls if significant resistance is noted.The investigation determined the reported steerable guide catheter cable break, resulting in noise and the inability to curve guide appears to be related to user error due to applying extreme curve on the guide.Based on the information reviewed, there is no indication of a product quality issue with respect to design, manufacture or labeling of the device.
