Event Description
It was reported that during the primary hip surgery the liner would not fit into the shell.A second liner was used, and the same issue occured.The surgery was delayed by more than thirty minutes.Attempts have been made and additional information on the reported event is unavailable.

This follow-up report is being submitted to relay additional information.If any further information is found which would change or alter any conclusions or information, a supplemental will be filed accordingly.Zimmer biomet will continue to monitor for trends.

This follow-up report is being submitted to relay additional information.Complaint sample was evaluated and the reported event was confirmed.Visual inspection identified several nicks around the rim and scuffs to the articulating surface.The locking flange and scallops have a few nicks and indents from the impaction and removal.Dhr was reviewed and no discrepancies relevant to the reported event were found.Review of the complaint history determined that no further action is required.Root cause was unable to be determined as the necessary information to adequately investigate the reported event was not provided.If any further information is found which would change or alter any conclusions or information, a supplemental will be filed accordingly.Zimmer biomet will continue to monitor for trends.
