|
It was reported that the patient underwent an initial hip surgery.During the procedure, poly inner component would not seat in the outer, metal shell.Another device was used to complete the procedure without patient injury.

(b)(4).This follow-up report is being submitted to relay additional information.Complaint sample was evaluated and the reported event was confirmed.Visual inspection was performed.Upon visual inspection the locking ring has been bent, the shell has visible signs of damage, the liner is missing scallops and shows visible signs of damage.Device history record (dhr) was reviewed and no discrepancies relevant to the reported event were found.Root cause was unable to be determined.If any further information is found which would change or alter any conclusions or information, a supplemental will be filed accordingly.Zimmer biomet will continue to monitor for trends.
