(b)(4).Per the manufacturer's subsidiary, the unit was being reset for the setup of the date when the issue occurred.
 
Event Description
It was reported that during set-up of the device for a cardiopulmonary bypass (cpb) procedure, the central control monitor (ccm) was black and would not activate.As a result, an alternate device was employed.The surgical procedure was completed successfully.There was no delay, no blood loss, nor adverse consequences to the patient.
 
Manufacturer Narrative
The reported complaint was confirmed.During laboratory analysis, product surveillance technician (pst) observed the central control monitor (ccm) to power on with a black screen and a "disk boot failure, instert system disk and press enter" error messaged displayed.The unit was rebooted five times, each resulting with the same error message.The complimentary metal-oxide semiconductor (cmos) battery had a volt reading of.2 (normal voltage is three volts).Information was received from the manufacturer's subsidiary that the batteries and hard drive of the ccm have never been replaced.It is recommended in the service procedure that the batteries and hard drive are replaced every six years.The product will be sent to service to be brought to manufacturer¿s specifications before being returned to the customer.If additional information becomes available on this complaint that would alter the facts and/or conclusion, a supplemental report will be filed accordingly.
