Upon receipt at our post market quality assurance laboratory, a thorough evaluation of the device was performed.A visual inspection of the device header and case noted no anomalies.The device was then exposed to simulated heart load conditions, and the defibrillation, pacing, and sensing functions were tested.Impedance testing was completed and all measurements were within normal limits.The device operated appropriately with no interruptions in therapy output at the returned programmed settings.A series of electrical tests was also performed, and again, normal device function was observed.
 
Event Description
Boston scientific received information that during an upgrade procedure the ventricular fibrillation (vf) was induced several times and the patient converted on their own.The cardiac resynchronization therapy defibrillator (crt-d) then went into post-shock pacing.The field representative stated they immediately tried to induce again and the device didn't detect the vf.It was also noted that post shock pacing was seen, however it was not actually pacing.The crt-d was attempted and not implanted due to undersensing.Boston scientific technical services (ts) was consulted after the case and discussed post therapy pacing parameters were likely active.Ts explained that it is driven by post shock pacing timing parameter which is nominally 30 seconds.No adverse patient effects were reported.
