The actual device was returned for evaluation.Reliability engineering evaluated the device and the reported condition was confirmed.A visual and functional assessment was performed which found that the device had low power, ran very slow and was sluggish.It was further observed that the device was not functioning with the epd-console.During service/repair, it was determined that there was damage insulation on the electronic control unit (ecu)-wire, the magnet from the control lever came out and the bearings and seals were worn out.The assignable root cause was determined to be due to normal wear out from use.If information is obtained that was not available, a follow-up medwatch will be filed as appropriate.

It was reported that during pre-surgery, it was discovered that the small battery drive device was not holding the charge well, had low power and was running sluggishly.During in-house engineering evaluation, it was observed that the magnet from the control lever came out.It was not reported if there were any delays in a surgical procedure or if a spare device was available.There was no patient involvement reported.There were no reports of injuries, medical intervention or prolonged hospitalization.The exact date of this event was unknown.All available information has been disclosed.If additional information should become available, a supplemental medwatch report will be submitted accordingly.
