Event Description
It was reported that the patient received an inappropriate shock for atrial fibrillation (af) with rapid ventricular response.It was also observed the patient had ventricular tachycardia (vt) and felt dizzy.It was noted the atrial fibrillation (af) with rapid ventricular response and ventricular tachycardia (vt) were induced by hypokalemia.Electrolytes and antiarrhythmic medications were administered.No action has been taken pertaining to the device and the device remains in use.The patient is a participant in the post approval clinical surveillance product surveillance registry.No further patient complications have been reported as a result of this event.

Event Description
It was further reported that the patient again received an inappropriate shock for af with rapid ventricular response.Antiarrhythmic medication was administered.It was noted the patient underwent an atrial ventricular node ablation and programming changes were made post ablation.
