(b)(4) is submitting on behalf of the foreign manufacturer, tosoh corporation, per exemption number (b)(4).Device evaluation by manufacturer: on (b)(6) 2018, a field service engineer followed-up over-the-phone with the customer.Fse asked customer to prime buffer pumps, the customer found leak at the union from the prefilter housing.Fse asked customer to remove and reconnect union.Customer primed buffers and no leak found.The g8 instrument is operating as expected.No further action was required by the fse.A 13-month complaint history review and service history review for similar complaints was performed for serial number (b)(4) from (b)(6) 2017 through aware date (b)(6) 2018.There were no other similar complaints identified during the searched period.The g8 service manual under chapter 6, assembly and disassembly, provides step-by-step instructions on disassembly and connection of parts surrounding the pump head which includes the filter housing and tubing.The most probable cause of the reported event was due to loose fitting.

On (b)(6) 2018, a customer reported that the line that goes into the filter housing is leaking on the g8 instrument.Technical support specialist (tss) had customer trace the line back as far as possible and it does go into a fitting where the leak is occurring.Customer is unable to tighten the fitting.A field service engineer (fse) was dispatched to address the reported event, which resulted in delay in reporting of patient results for hemoglobin a1c (hba1c).There is no indication of any patient intervention or adverse health consequences due to the delay in reporting of patient results.
