Event Description
|
It was reported to boston scientific corporation that a lynx system was used during a suprapubic mid-urethral sling procedure.According to the complainant, during the procedure, upon placement of the sling, the blue covering of the trocar was not tight and it came loose.The procedure was completed with another lynx system.There were no patient complications reported as a result of this event.All other information is unknown.Should additional relevant details become available; a supplemental report will be submitted.

A visual examination of the returned lynx system revealed that the blue covering on the delivery device was loose and could be completely removed.The metal shaft was bent which most likely caused the blue covering to become loose.Analysis also revealed no damage to the mesh assembly.A review of the device history record (dhr) confirmed that the device met all material, assembly, and product specifications at the time of release to distribution.The investigation concluded that this complaint is associated with a product that meets the design and manufacture specifications but due to anatomical/procedural factors encountered during the procedure, performance of the device was limited.The most probable root cause classification is operational context.
