Concomitant medical products: collamer ultraviolet-absorbing posterior chamber three piece foldable intraocular lens.Visual inspection found a bent haptic, tear on the optic and portion of the optic missing.No similar complaint type events were reported for units within the same lot.(b)(4).
 
Event Description
A loading issue with a cq2015a, +19.5 diopter, intraocular lens was reported.The reported stated that there was patient contact with no patient injury.Additional information has been requested but none has been forthcoming.If additional information is received a supplemental medwatch report will be submitted.
