(b)(4).During processing of this complaint, attempts were made to obtain complete event, patient and device information.The device was not returned for evaluation.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no other similar incidents from this lot.The investigation was unable to determine a conclusive cause for the reported complaints.There is no indication of a product quality issue with respects to the design, manufacture, or labeling of the device.
 
Event Description
It was reported that the procedure was to treat an 80% stenosed lesion in the circumflex.A 3.75x8mm nc trek balloon dilatation catheter (bdc) was being used to post-dilate a deployed non-abbott stent.The contrast mix was 50:50 and the balloon was inflated 2 times at 14 atmospheres.Difficulty deflating the balloon was noted and the balloon prolapsed on the proximal end when it was deflated.The bdc was removed with the balloon deflated.There was no clinically significant delay in the procedure and no adverse patient effects.No additional information was provided.
