(b)(4).During processing of this complaint, attempts were made to obtain complete event, patient and device information.The device is expected to be returned for evaluation.It has not yet been received.A follow up report will be submitted with all relevant information.The xience sierra is currently not commercially available in the u.S.; however, it is similar to a device sold in the u.S.
 
Event Description
It was reported that the procedure was to treat a heavily calcified lesion located in the distal right coronary artery.A 3.5 x 38 mm xience sierra stent delivery system (sds) could not advance to the target lesion with the guiding catheter.The sds was removed from the guiding catheter with slight resistance when the distal struts became lifted [flared].The procedure was successfully completed with a non-abbott stent and a guideliner.There was no adverse patient effects and no reported clinically significant delay in the procedure.No additional information was provided.

(b)(4).The device was returned for analysis.The reported difficulty to position, difficulty to remove and material deformation (stent flare) were confirmed.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no other incidents from this lot.The investigation determined the reported difficulty appears to be related to circumstances of the procedure.There is no indication of a product quality issue with respect to manufacture, design or labeling.
