Event Description
A customer reported that the system froze during a procedure and the footswitch cable had a blister.Another system was used to complete the procedure.There was no patient impact reported.
 
Manufacturer Narrative
The system was examined and the reported event was not replicated.However, the footswitch cable (which belongs to the system) was replaced (and returned for evaluation), as a preventive measure.The system was manufactured on june 24, 2014.Based on qa assessment, the product met specifications at the time of release.A footswitch cable was received for evaluation.A visual assessment of the returned sample revealed a bump in the cable near the footswitch-end connector.The sample was installed onto a calibrated system for functional testing.The sample was found to meet relevant specifications.The system was found to meet specifications.Therefore, the root cause of the reported freezing issue cannot be determined conclusively.Although the cable was found to be visually nonconforming, the sample was found to meet relevant specifications when testing.Therefore, the root cause of the footswitch cable issue cannot be determined conclusively.(b)(4).
