|
It was reported that during use of the device for a cardiopulmonary bypass (cpb) procedure, the device had a level sensor disconnection alarm.As a result, an alternate device was employed.The surgical procedure was completed successfully.There was no delay, no blood loss, nor adverse consequences to the patient.

Per clinical review: on (b)(6) 2018, the clinicians had a red alarm level sensor disconnection message.The team re-connected the cable to the pad and the alarm continued.The team did not change out the cable and opted to use only the alert sensor for the remainder of the procedure.The team used gel for their level sensors, and does wait the instruction for use (ifu) suggested five minutes prior to loading the level sensor cables in the level sensor pads.The team does use a flat jms reservoir and does place the level sensor pads directly on the reservoir.According to the information available, the team exchanged the level module after the procedure and the system performed as expected.There was no delay in the surgical procedure of the event with the level alarm sensor.There was no blood loss or harm associated with the event.
