Event Description
|
It was reported that during set-up of the device for a cardiopulmonary bypass (cpb) procedure, while calibrating the occluder head, there was a noise coming from it and there were also screws missing.The electronic occluder was replaced for a manual occluder.The surgical procedure was completed successfully.There was a delay of five minutes.There was no blood loss nor adverse consequences to the patient.

During laboratory analysis, the product surveillance technician (pst) observed a louder than normal noise coming from the motor of the occluder head, especially at full occlusion.The product will be sent to service to be brought to manufacturer¿s specifications before being returned to the customer.If additional information becomes available on this complaint that would alter the facts and/or conclusion, a supplemental report will be filed accordingly.

Event Description
|
Per clinical review: during set up for a cardiopulmonary bypass (cpb) procedure on (b)(6) 2018, the clinicians had an incident with the venous occluder.When the clinician was calibrating the occluder there was a high pitched noise coming from inside the occluder, therefore they deemed the occluder head to be malfunctioning and opted to exchange the occluder out.The delay in set up was approximately five minutes.There was no blood loss or harm associated with the delay.
