Event Description
|
It was reported that the patient presented to the emergency room after receiving a vibratory alert.Interrogation revealed that during capacitor maintenance, the device exceeded its maximum charge time.Two additional capacitor maintenance tests were performed.Both were within the charge time, but one was significantly longer than the other.No intervention was performed.The patient was stable and will be monitored.

Additional information: d10.Correction: b1 - adverse event, b2 - required intervention to prevent permanent impairment/damage (devices), and h1 - serious injury should have previously been selected in the previous report.The extended charge time was confirmed via reviewing of the device image.Bench testing was performed, and the device subassembly showed normal characteristics.The high voltage capacitors were analyzed and confirmed a failure of a capacitor.Extended charge time was caused by an anomalous hv capacitor.
