Event Description
It was reported that during the implant procedure, the implanter initially tried to place the left ventricular (lv) lead but had difficulty getting the lead into the desired vessel.It was noted when the lead was removed from the sheath, the physician visualized blood on the inside of the lead.The physician requested the lv lead be replaced and was not implanted.No patient complications have been reported as a result of this event.

Product event summary the full lead was returned, analyzed, and no anomalies were found.The outer insulation of the lead was observed to have blood ingression.Visual analysis of the lead indicated damage at implant.The analyst noted that there is blood ingression at the distal end of the lead which is normal for this model of lead.If information is provided in the future, a supplemental report will be issued.
