It was reported that, during the implant procedure, a right ventricular (rv) lead could not be inserted into this cardiac resynchronization therapy defibrillator (crt-d).The physician observed that there was an obstacle near the set screw hub.The sets screw was fully retracted but lead insertion was still unsuccessful.The physician requested a new device, however none were available.The pocket was closed and an additional procedure was performed the next day to implant a new device.No additional adverse patient effects were reported.
