This alleged failure mode poses a low risk to the patient because the freedom onboard battery was not supporting the patient.The freedom onboard battery will be returned to syncardia for evaluation.The results of the investigation will be provided in a follow-up mdr.(b)(4).

Event Description
|
The freedom onboard battery was not in use by a patient.The customer, a syncardia authorized distributor, reported that freedom onboard battery was stuck in the freedom driver and could not be removed initially.After several attempts the patient was able to remove the battery.

|
The freedom driver in use at the time of the customer-reported issue was also returned with the freedom onboard battery.An onboard battery exchange test was performed using the freedom driver, the onboard battery and four test onboard batteries where all were inserted and removed without any difficulties or abnormalities.There was no evidence of a device malfunction and the root cause of the customer-reported issue could not be conclusively determined.This issue will continue to be monitored and trended as part of the customer experience process.Syncardia has completed its evaluation of this complaint and is closing this file.(b)(4) follow-up report 1.
