(b)(4).Customer has indicated that the product has returned to zimmer biomet for investigation.The investigation is in process.Once the investigation has been completed, a follow-up mdr will be submitted.
 
Event Description
It was reported that there was a piece of debris on top of the foam underneath the sterile sealing while the implant was opened.Attempts have been made and additional information on the reported event is unavailable at this time.No further information is available.
 
Manufacturer Narrative
(b)(4).This follow-up report is being submitted to relay additional information.Complaint sample was evaluated and the reported event was confirmed.Reported event was confirmed by evaluation of returned product.Visual examination of the returned product determined the debris to be from the foam packaging.Device history record (dhr) was reviewed and no discrepancies were found.The likely condition of the products when they left zimmer biomet was conforming to specifications.The root cause of the reported event is likely to be due to transit damage causing the foam to shed.A summary of the investigation has been sent to the complainant.If any further information is found which would change or alter any conclusions or information, a supplemental will be filed accordingly.Zimmer biomet will continue to monitor for trends.
