Event Description
It was reported that following an explant procedure (mfr: 3006705815-2019-03423) a piece of the explanted anchor was missing.Upon review of the pre-op x-rays, the anchor was broken in two.Physician elected to not go back in to remove the fragment.
 
Manufacturer Narrative
 the reported broken anchor was visually confirmed.Analysis of the returned anchor found that the distal end of the silicone overlay was missing.
