Date of event was approximated to (b)(6) 2019, the surgical removal date of the mesh, as no event date was reported.The complainant was unable to provide the suspect device upn and lot number; therefore, the lot expiration and device manufacture dates are unknown.(b)(4).(b)(6) adverse incident report reference nos.(b)(4); submitted to (b)(6) by the physician.The complaint device is not expected to be returned for evaluation; therefore, a failure analysis of the complaint device could not be completed.If any further relevant information is identified, a supplemental medwatch will be filed.
 
Event Description
It was reported to boston scientific corporation that an obtryx system and a mesh from a different manufacturer were implanted during a tot (trans-obturator tape) and tvt (tension-free vaginal tape) procedure performed on an unknown date.According to the complainant, after the procedure, the mesh infiltrated in the bladder wall, urethral sphincter and damaged the adductor longus muscle.Subsequently, the patient experienced chronic pelvic pain.On (b)(6) 2019, the patient underwent surgical removal of vaginal component of tot and tvt mesh and reconstruction of urethra and vagina.The patient's current condition is unknown.
