The product in complaint was not returned to the manufacturer for evaluation.A review of the manufacturing records for this device was completed and no issues were identified that could have lead to the adverse event reported.Complaints will continue to be monitored for any trends.
 
Event Description
It was reported that an asymptomatic female patient with a focal calcified lesion at bifurcation underwent a transcarotid artery revascularization (tcar) procedure and suffered a dissection in the left common carotid artery (lcca).This occurred while placing the arterial sheath.There was no blood return once sheath was placed, so the physician pulled the arterial sheath back until there was sufficient back bleeding.The physician performed an arteriogram after the arterial sheath was secured and a dissection in the distal lcca was observed.The physician used a 7x40 enroute stent to cover the lesion, which landed distally in the left internal carotid artery (lica) and proximally in the lcca.The physician then placed a 8x30 enroute stent proximally in the lcca to cover the dissection flap.The procedure was performed under local anesthesia.All vitals were reported to be normal and the patient remained neuro intact throughout.No additional details were provided.
