Patient's exact age is unknown; however, it was reported that the patient was over the age of 18.(b)(4).The complaint device was disposed at the facility; however, the photo sent in by the customer showed the ligator head had five bands present which were moved out of their original positions, with one band was caught under other bands, confirming that the bands failed to deploy.It was also noticed that the ligator teeth were bent.This failure is likely due to factors or conditions related to procedure during the use of the device that could have affected its performance and its intended purpose.Therefore, the most probable root cause is adverse event related to procedure.A review of the device history record (dhr) was performed and confirmed that this device met all material, assembly and performance specifications.

It was reported to boston scientific corporation that a speedband superview super 7 device was used in the esophagus during a band ligation procedure performed on (b)(6) 2019.According to the complainant, during the procedure, the bands could not be released.Reportedly, after removing the scope from the patient, the bands were found to be overlapping.A photo sent in by the customer confirmed the alleged issue.The procedure was completed with another speedband superview super 7 device.Additionally, there was no difficulty in setting up the device.There were no patient complications reported as a result of this event.The patient condition at the conclusion of the procedure was reported to be good.
