|
The recipient reportedly experienced loss of lock.External equipment was exchanged, however, the issue did not resolve.The recipient's device was explanted.The recipient was reimplanted with another advanced bionics cochlear device.

Advanced bionics considers the investigation into this reportable event as closed.The device passed the external visual inspection.The photographic imaging inspection revealed cut electrode wires.This is believed to have occurred during revision surgery.System lock could not be obtained at any spacing.The device passed an electrical test performed.The device passed the mechanical tests performed.It is believed that the failure of this device is most likely due to a malfunction within the digital chip.Internal visual inspection did not reveal any anomalies on internal components.This older device configuration is not currently manufactured.This is the final report.Disclaimer: advanced bionics does not intend that this report be any admission of liability, fault or product defect.
