The customer, a syncardia certified hospital, reported that the freedom driver exhibited a fault alarm upon startup while supporting the patient.The customer also reported that the patient was subsequently switched to a backup freedom driver and that the patient was fine during the driver switch.

The driver's alarm history was reviewed and revealed one fault alarm, a 28 alarm code, which is produced when the driver's mpu detects a voltage out of the predetermined accepted values for lcd regulator 1.The driver passed all sections of functional testing.Additionally, since the recorded alarm is related to a change in voltage, an onboard battery exchange test using four different freedom onboard batteries was also performed in an attempt to replicate the customer-reported issue.During this test, no alarms were produced.The customer-reported issue could not be reproduced.The driver performed as intended with no evidence of a device malfunction.The cause of the customer-reported fault alarm could not be conclusively determined.This issue will continue to be monitored and trended as part of the customer experience process.Syncardia has completed its evaluation of this complaint and is closing this file.(b)(4)follow-up report 1.
