This alleged failure mode poses low risk to a patient because the issue was observed when the freedom driver was not supporting a patient.The freedom driver will be returned to syncardia for evaluation.The results of the investigation will be provided in a follow-up mdr.(b)(4).
 
Event Description
The freedom driver was not supporting a patient.The customer, a syncardia certified hospital, reported that both of the freedom onboard batteries were unable to latch in the freedom driver.
 
Manufacturer Narrative
Visual inspection of the driver revealed fractured plastics, battery safety latch stuck in the pressed down position and a battery safety latch spring out of the correct position.During investigation testing, the customer-reported issue was reproduced as the onboard batteries were unable to latch in the freedom driver.The root cause was determined to be that the right battery safety latch and spring were stuck due to the safety latch spring out of the correct position.It could not be conclusively determined how the spring on the right battery safety latch moved out of the correct position.Once the spring was repositioned correctly, the freedom driver passed all sections of functional testing.This issue will continue to be monitored and trended as part of the customer experience process.Syncardia has completed its evaluation of this complaint and is closing this file.(b)(4) follow-up report 1.
