On (b)(6) 2019, patient underwent bronchoscopic procedure and three zephyr valves (4.0 ebv, 4.0-lp ebv) were placed in the right upper lobe (rul) and right middle lobe (rml).Within 30 minutes of zephyr ebv placements, patient developed shortness of breath.Chest x-ray revealed large right pneumothorax.Physician immediately inserted pigtail catheter and patient subsequently had improvements in signs and symptoms and chest x-ray.On (b)(6) 2019, the zephyr valve in the rml was removed, and the patient healed by the next day.On (b)(6) 2019, the chest tube was removed and the patient was discharged.
