The freedom driver was not supporting a patient.The customer, a syncardia certified hospital, reported that the freedom driver did not pass system check out procedure because the driver did not start when putting the battery in and the display showed asterisks and after a short while enunciated an alarm.

Visual inspection of the driver revealed that the driver's secondary motor's cam follower was out of the bottom dead center (bdc) position.This is typically indicative of secondary motor engagement.The driver's alarm history was reviewed and revealed an alarm, a '12' code, which is produced by the top piston cylinder assembly (pca) sensor not detecting any movement for five seconds.After a '12' alarm, the secondary motor will always engage.During investigation testing, the customer-reported issue was able to be reproduced as the primary motor did not engage and the driver functioned as intended by switching to the secondary motor system and enunciating an alarm as designed.The root cause was determined to be the inability of the main printed circuit board assembly (pcba) to operate the primary motor.This issue will continue to be monitored and trended as part of the customer experience process.Syncardia has completed its evaluation of this complaint and is closing this file.(b)(4) follow-up report 1.
