Event Description
It was reported that the patient presented with a urinary tract infection.Blood cultures were taken and the organism was noted as s treptococcus agalactiae.A transesophageal echocardiogram (tee) was performed and vegetation was observed on the right atrial (ra) lead.It was further reported the patient had endocarditis.The cardiac resynchronization therapy defibrillator (crt-d) system was explanted and the patient was treated with antibiotics.The patient is a participant in the adaptresponse clinical study.No further patient complications have been reported as a result of this event.

Event Description
It was further reported that the patient went to the emergency department with chest pain with nausea, vomiting, and diarrhea.The patient was admitted for signs and symptoms of an infection at which point the blood cultures and tee were taken.A temporary pacemaker was placed and vegetation debulking was performed.
