Event Description
During an in clinic follow-up, noise resulting in oversensing was noted on the device. The suspected cause of the event was a header anomaly that prevented the right ventricular (rv) lead from being inserted adequately. The device was explanted and replaced to resolve the event. The patient was in stable condition.

Manufacturer Narrative
The reported oversensing was confirmed from the egm review, however, the device was revealed to be normal. The right ventricular (rv) oversensing seen in device egms was consistent with the rv lead insulation damage reported on a competitor's product by the field. This should be seen as a circumstance outside of the devices control and should not be considered an anomaly.During functionality testing, test leads were successfully inserted and able to be properly secured into the header.Furthermore, telemetry, impedance, sensing, pacing and high voltage (hv) output functions of the device were tested and revealed to be normal.
