Event Description
|
It was reported that during a laparoscopic colectomy, the surgeon felt something unusual when the clip was fed into the jaws for the first firing, and a tear drop-shaped clip was formed while feeding the clip.The device was used on the blood vessel.After that, the surgeon tried to feed a clip into the jaws outside the patient for functionality, but the issue was not resolved.Another device was used to complete the case.There were no adverse consequences to the patient.

(b)(4).Date sent: 1/17/2020.D4: batch # t94u28.Investigation summary: the analysis results found that the el5ml device was returned with no damage in the external components.In an attempt to replicate the reported incident, the instrument was tested for functionality.During the analysis, the device was cycled and it fed and formed 7 conforming clips.The event described could not be confirmed as the device performed without any difficulties noted.The reported complaint could not be confirmed.A manufacturing record evaluation was performed for the finished device batch number, and no non-conformances were identified.
