It was reported that three units of stopcocks had cracked during use.There was no patient injury or medical intervention associated with this event.No additional information is available.

Additional information :additional information : the actual and video of the sample was received for evaluation.A visual inspection was performed using the naked on the actual sample which revealed a crack in the luer.The video sample underwent visual inspection which revealed the stopcock was leaking from the middle inlet.The stopcock is not intended for use with syringe and it was not possible to insert the syringe fully as the customer had done.The reported condition was verified.The cause of the condition could not be clearly determined.A batch review was conducted and there were no deviations found related to this reported condition during the manufacture of this lot.Should additional relevant information become available, a supplemental report will be submitted.
