Evaluation summary: post market vigilance (pmv) led an evaluation of one device.The instrument was received partially applied.No visual abnormalities were observed with the instrument.The instrument was cycled and the clip did not load, the pusher bar was observed not functioning properly.A review of the device history record indicates this product was released meeting all quality release specifications at the time of manufacture.However, a manufacturing fault was identified during product analysis.The root cause of the observed condition was determined to be a result of a manufacturing activity.The trip lever out of position was preventing the clips from loading properly.Improvements have been initiated to mitigate this condition.If information is provided in the future, a supplemental report will be issued.

According to the reporter, during laparoscopic cholecystectomy procedure, the clips were able to load properly into the jaws, but when the surgeon attempted to fire the first clip it did not fire.The surgeon was not able to squeeze the handle and once the 2nd clip was loaded the jaws locked and the device could not be fired.The surgeon then used another device to resolve the issue in order to complete the case.There was no patient injury.
