Event Description
|
The syncardia companion hospital cart is a large cart with wheels into which the syncardia companion 2 driver docks.It is intended for use in the hospital during the temporary total artificial heart (tah-t) implant procedure and subsequent recovery.The customer, a syncardia certified hospital, reported that the screen on the hospital cart turned blue while supporting a patient.

During investigation testing, the customer-reported issue of the blue hospital cart screen was only able to be reproduced when a companion 2 driver was intentionally docked improperly into the hospital cart and missed the docking guide.This prevented proper electrical connection for docking the driver into the cart and caused the cart screen to change to a blue color.The companion 2 driver operator manual states in section 1.2 that the driver docks into the hospital cart by placing the driver straight into the center of the hospital cart so the docking connectors mate.This issue will continue to be monitored and trended as part of the customer experience process.Syncardia has completed its evaluation of this complaint and is closing this file.Ce 5211 follow-up report 1.
