Event Description
It was reported that tubing free flowed, causing an over infusion of insulin.The event occurred while inserting and removing the tubing from the pump.As a result of the event, patient experienced hypoglycemia and blood glucose dropped from baseline 123 to less than 18.50% dextrose was administered and blood glucose increased from less than 18 to greater than 374.As a result of the event, frequent blood glucose monitoring was required.
