Event Description
According to the reporter, during use, a hairline crack was noted on the luer adapter causing a leak.The catheter was repaired with a repair kit.Tego was not utilized; no instrument was used to tighten and loosen the adapters.Octenisept was the cleaning agent used.It was stated that bd q-syte connectors were used and this could be the reason why the luer adapter broke after a time of use.There was no reported patient injury.
 
Event Description
