Visual analysis was performed on the returned device.The reported broken tip was not confirmed.The tip was not nearly detached as reported; however, the inner member was kinked and twisted proximal to the base of the tip which was likely the reported tip damage.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no other complaints.The investigation determined that inadvertent mishandling prior to use or during removal from the packaging likely caused the damage to the inner member proximal to the tip.There is no indication of a product quality issue with respect to the design, manufacture, or labeling of the device.
