Device evaluated by mfr.: the device was returned for analysis.Visual inspection showed a detachment of the imaging window from the lapjoint section was observed.The imaging window did not returned for analysis.Multiple kinks were observed along the sheath assembly.No other damages were encountered upon visual inspection.
 
Event Description
Reportable based on device analysis completed on 04jun2020 it was reported that lost image occurred.The 85% stenosed lesion was located in the severely tortuous and moderately calcified left anterior descending artery.An opticross imaging catheter was selected for use.During preparation it was noted that the catheter could be recognized by the motor and the system, but there was no image showed from the beginning.The procedure was completed with another of the same device.No patient complications were reported.However, device analysis noted a detachment of the imaging window from the lapjoint section.
