Event Description
|
According to the reporter, during a procedure, staff scanned patient with mat and the system detected a sponge.Count was correct.Staff rescanned with wand and no sponge was detected and they rescanned with mat once again and not sponge detected.There was no patient injury.

According to the reporter, during breast lumpectomy and sentinel node biopsy, staff scanned patient with mat and the system detected a sponge.Count was correct.Staff rescanned with wand and no sponge was detected and they rescanned with mat once again and no sponge detected.Nurse continued trying until she obtained a negative results on both matt and the wand.There was no patient injury.
