According to the reporter, prior to a procedure, while attempting to hand the device off (onto) the sterile field, it was noted that the distal tip of the black suction tube was flared and damaged, with a piece of metal turned up in a sharp and pointy manner.The device was not introduced to the sterile field.Another device was used to resolve the issue.No patient involvement.
