The actual complaint product was not returned for evaluation.Root cause could not be determined.A review of the device history record is not possible as no lot number was provided.All information reasonably known as of 17 aug 2020 has been included in this health authority report.Should additional information be obtained, a follow-up health authority report will be provided.The information provided by avanos medical inc.Represents all of the known information at this time.Avanos medical inc.Has no independent knowledge of the event reported but is relaying the information that was provided by the user facility where the incident occurred.This product incident is documented in the avanos medical complaint database and identified as complaint (b)(4).
 
Event Description
It was reported that a foreign fragment was found in the catheter.Once removed it is determined to be a small piece of plastic.The tube was inspected and additional plastic pieces were noted in the suction catheter.The closed suction device was removed from the patient.It was reported that "at the top of the tracheal tube broken off pieces of plastic sitting lightly glued to the inside of the tracheal.The child side-mounted and the tube facing downwards to prevent the pieces fall into the tracheal tube, and the stumps pry out with a needle two of the smallest pieces however, falls further down the tube and one is attached on the tracheal tube to prevent the pieces falls into the patient." additional information was requested, however, hospital was unable to provide any further details.
