The device was returned to olympus for evaluation.The user¿s complaint was confirmed.The device was inspected and found no suction flow, a foreign object at the 50cm level and a worn insertion tube due to multiple buckles and reduced angulation.The listed parts were replaced.The device was returned to full functionality and returned to the user facility.

The user facility reported that an aspiration problem occurred while the device was in use during a procedure.The scope was in the patient¿s mouth.The physician could pass anything down the scope as there was something stuck in it.There was no patient injury or harm.No additional information was provided.
